Filename as Headline

Description

The name of the (partial) included file should be displayed by default as a headline direct above the source code. The user should be able to overwrite this by using an optional argument, e.g. header=Text, where an empty text disables the headline.

Also added ability to specify header_class parameter to override default CSS class of 'content' (which is what mimeview defaults it to).

The resolution will be deleted. Next status will be 'reopened'.
