Pages without titles (such as =TitleName=) cause the generation of a 0 byte PDF in multi-page PDFs

Description

I suspect this is due to attempting to generate a TOC for the PDF w/ a null entry. However best approach would be to simply not include a TOC entry for that page.

The workaround is to add a title or remove that page when generating multi-page PDFs
