looks like a bug in email parsing, I just replied, no base64 encoded stuff attached

I ran into this problem as well with Trac instances that do have smtp_subject_prefix defined. In this case, Trac 0.11.6 defaults to a value of __default__ which this plugin doesn't handle.
I'll attach a quick patch to handle the default value. I couldn't find an API in Trac 0.11.6 to return the actual subject prefix, so I had to determine it manually.

ReplyToTicket also fails to recognize replies to tickets if trac.ini
contains a custom ticket_subject_template
with a variable other than $prefix, $ticket.id and $summary.
