Improve CSS and page design

The current presentation is space wasting and overloaded.

Try to improve by rearranging items and tweaking CSS styles. Folding page sections may also be considered.

PeerReviewPlugin: show user names instead of login names when selecting reviewers. Make Cancel button default in confirm dialog when removing users. Some CSS tweaking to make comment tree denser.
